In order to mitigate motor vehicle collisions involving teenagers, a specialized "provisional" driver license (DL) and instruction permit are allocated to individuals under the age of 18. To acquire a provisional instruction permit, the following criteria must be met:

The applicant must be at least 15 1/2 but under 18 years old.Complete the Driver License or Identification Card Application (DL 44) form, which includes providing your social security number if eligible.Obtain the signatures of parents or guardians on the DL 44 form.
